We aim to ensure that our employees recieve first class training and supervision to deliver these services in a friendly and professional manner. Our goal is to continually stive to be the best, safest, and most reliable provider of these services to our patients and community.

Admission Criteria

For Admission to Our Facility A Patient Must:

Admission Criteria
  • Require the active and ongoing therapeutic intervention of multiple therapy disciplines (physical therapy, occupational therapy, speech-language pathology, or prosthetics/orthotics), one of which must be physical or occupational therapy
  • Generally require an intensive rehabilitation therapy program
  • Patient must be able to participate and benefit from an individualized therapy plan
  • Be at least 19 years of age with medical necessity for inpatient rehabilitation

At Pathway Rehabilitation Hospital, we are devoted to helping our patients regain their independence after sustaining a functional loss due to a disabling injury or illness which includes but not limited to the following:


Cerbrovascular Accident/Stroke

Neurological Disorders

(including Multiple Sclerosis, Parkinson’s Disease, and Polyneuropathy)


Fracture of the Femur

(Hip Fracture – Femoral Neck)

Joint Replacement

(bilateral, age > 85, body mass > 50)

Major Multiple Trauma

Spinal Cord Injury

Brain Injury

Congenital Deformity

Other Orthopedic Disorders

Other Disabilities
